The South Texas APS Community Board and Silver Sabbath Committee are hosting their second annual symposium bringing together presenters from aging services who will share their wisdom, insight and provide relevant networking opportunities.
Championing Connections…Aging Well Together Symposium will be an opportunity to plan and learn about essential community resources beneficial aging community members. Participants will receive information on legal issues and important documents that older adults and/or family caregivers should seriously consider having in place.

Making the Move – Preparing for a Transition in Care. Presented by Byron Cordes, LCSW, C-ASWCM, CMC Certified Aging Life Care Professional | Sage Care Management.
· Description – Deciding to get help or move from home to another level of care is a stressful decision. We will discuss what preparations you can make now to make that decision easier later. We will go over trigger points, costs, and what you will want or need from family and professionals.
Get The Most out of Medicaid STAR+PLUS. Presented by James B Ybarbo, Maximus Texas Health – EB Outreach
· Description – This session will explain how Texas contracts with MCOs to deliver long term service supports. Managed Care is a health care delivery system organized to manage cost, utilization, and quality. Learn how Medicaid managed care provides for the delivery of Medicaid health benefits and additional services through contracted arrangements between the Texas Medicaid agency and MCOs.
Need Assistance Applying for Benefits? Meet A Panel of Community Experts Ready to Help!
· Description – Finding help for older adults and people with disabilities can be overwhelming. Aging and Disability Resource Centers (ADRC) were created to simplify access to resources by providing one point of contact for people who need help. This distinguished panel will also include other organizations who can help older adults make connections to community services that can help and maintain good health.
Important Legal Matters for Caregivers. Presented by Paul Zambie, Texas Legal Services
· Description – The session will cover programs and documents that are useful when planning for elder years. The speaker will discuss Medicaid eligibility and will outline the benefits of the Qualified Income trust, which can help people qualify for Long Term Medicaid as well as the Transfer on Death Deed, which can help avoid the costs associated with probate and exempt the property from Medicaid estate recovery (MERP). Other topics covered include Powers of Attorney and Advance Directives.
Adult Protective Services Is Here to Help! Understanding the Truth Behind APS Policies and Procedures
· Description – Join the Adult Protective Services Leadership team as they discuss what APS can and cannot do. Learn the facts on how APS investigates cases and the different ways APS uses least restrictive alternatives when arranging services to prevent further harm. In the end, our aim is to care for Older Texans and adults with disabilities in a way that makes our clients feel secure, comfortable, and most importantly valued and respected.
